Puppy Training?

I have a Shitzu that is almost 1 year old. It was established kennel since he was 8 weeks. Every time I think finally managed to find a puddle of urine in my program i carpet. Does anyone have any idea? Past and must know that he knows better because he runs and hides when I are coming. I'm not sure what to do next. I tried to reward and praise, but slips and still pee in the house. Any ideas would be appreciated!
